Requesting Documentation for Illness or Missed Academic Work 

We understand that illness and personal challenges can impact your academic responsibilities. Here’s what you need to know if you're seeking documentation (Verification of Illness) from the Health & Counselling Centre (HCC): 

If you have a current relationship with a healthcare provider in the community, you are encouraged to see them for documentation. 

When We Can Provide Documentation: 

Verification of Illness (VOI) forms are issued, in person, on the same day you were assessed by one of our healthcare providers. This means that VOIs cannot be provided for illnesses that happened in the past. 
If you visited an emergency department on the day of illness, please bring a summary of your visit— we may be able to provide documentation based on the summary information 
If you are requesting documentation to support a petition or appeal with the Office of the Registrar, a VOI can be provided, when appropriate. You must have had a pre-existing care relationship with the HCC and have received care during the relevant time frame.

When We Cannot Provide Documentation: 

For any past illnesses, missed classes, or exams, if you were not assessed by us during the time of illness. 
VOIs are not provided over the phone. If you are unwell and requesting a VOI, you will be asked to come in person. There are very limited exceptions to this. 
If you arrive towards the end of the day and we have no remaining availability, you will be directed to a walk-in clinic to obtain your VOI. 
If you are registered with Accessibility Services, a VOI is not the appropriate form of documentation for missed work (ie: assignment extensions, missing coursework), if related to the documented pre-existing condition. You will need to connect directly with your Accessibility Advisor, or general Accessibility Services .
Note: If your instructor has asked you to provide a VOI, you should still connect with your Accessibility Advisor, as VOI’s are not intended for pre-existing conditions.  
If your current concern is unrelated to your registered condition with Accessibility Services and you are seen at the HCC on the day of illness, we may be able to provide a VOI as outlined in the section above.

Important Note: 

VOIs and support letters are based on the clinical assessment of HCC staff at the time of visit and are provided at their discretion. VOIs are not guaranteed. The HCC does not provide letters of support for petitions, however, please see notes above for situations where we may be able to provide a VOI.
