St. George and Scarborough students taking courses at the University of Toronto Mississauga (UTM) must first register with Accessibility Services on their home campus.
- Register with Accessibility Services at your home campus.
- Complete a Transfer of Accommodation Form and provide to your Accessibility Advisor (at your home campus).
- Complete the UTM Accessibility Services Consent Form and Online Intake form (Online Intake Word Document). Please attach consent form in the online intake.
- Log in to AIMS: https://aims.utm.utoronto.ca/custom/misc/home.aspx and select the Document Upload Portal Icon.
- Upload your completed and updated Medical Documentation to the Document Upload Portal. Accessibility Video Module on Document Upload Portal also available to review at the following link: https://www.youtube.com/watch?v=crP43Lai65Y
- Accessibility Staff will follow-up with you regarding next steps.
